Patient Resources
From the moment you schedule your appointment, we strive to provide you with quality medical eye care that each patient deserves.
Below are few of the resources that may be useful for you.
New Patients
Please try to arrive 15 minutes before your scheduled time and fill the new patient form before your appointment.
Also your appointment may take up to 2 hours. If we decide to dilate your pupils, your vision may be a bit blurry. You may want to bring someone to drive you home. If you have any questions about this, please call.
Payments
We participate with a wide variety of insurance plans. Please bring your insurance card(s) with you so we can verify that we have the correct information to bill your insurance company. You will be asked to pay for all non-covered services at the time of the visit.
Patients with co-pay requirements must pay at the time of their visit. If you are not prepared to pay, you may be asked to reschedule your appointment. We accept cash, Visa, Mastercard, and personal checks for payment.
Referral
If your insurance carrier requires you to have a referral, you must bring a copy with you. Without the referral, you may be asked to reschedule your appointment.
Cancellation Policy
We ask that you kindly let us know 24 hours in advance if you will not be able to make your appointment. There is a 40 dollar no show fee.
